I am requesting access to LHI’s Customer Portal for the purpose of verifying data housed by LHI. I certify that I have authority within the scope of my responsibilities to view the data contained in the LHI Customer Portal for a limited set of individuals who utilize LHI services. I agree to abide by the following terms and conditions.
I understand that when I am viewing data in the LHI Customer Portal, I may be exposed to Protected Health Information (“PHI”) as that term is defined and used in the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and “records” as that term is defined and used in the Privacy Act of 1974, 5 U.S.C. 552a (“Privacy Act”), and other information deemed to be confidential by other laws. PHI is information about a person’s health or treatment that identifies the person and as further defined in 45 CFR 160.103. A record means any item, collection, or grouping of information about an individual that is maintained by an agency, including, but not limited to, education, financial transactions, medical history, and criminal or employment history containing a name, or the identifying number, symbol, or other identifying particular assigned to an individual, such as a finger or voice print or a photograph.
In addition, I understand that “Confidential Information” may also be available to me when accessing the LHI Customer Portal. “Confidential Information” may include sensitive information relating to an individual’s data which is not considered PHI or a record.
I agree to use and disclose this PHI, record, Confidential Information and any other information, only for the limited purposes of my authorized responsibilities and otherwise to keep the information confidential. Any disclosures of this data shall not be provided to third parties. I understand that I will be only permitted to view and print information contained in the LHI Customer Portal as authorized by my job function. Electronically sending data received from the LHI Customer Portal outside of the approved systems and/or approved users is strictly prohibited. Electronically sending such data to a home or personal account is strictly prohibited. Sending any confidential, sensitive, or PHI data unencrypted is strictly prohibited.
I agree that I am responsible for ensuring that the electronic device I use to access the LHI Customer Portal has the appropriate information security protections, such as anti-virus software. I agree to safeguard my LHI issued user name and password which is needed to access the LHI Customer Portal. I understand that after five unsuccessful attempts of logging in to the LHI Customer Portal, I will be locked-out of the system and must contact LHI for a new password. LHI has the right in its sole discretion to deny or limit my access to the LHI Customer Portal at any time for any reason.
LHI’s systems are solely for the use of authorized users for official legitimate work purposes, with no expectation of privacy. The actual or attempted access, use, and/or modification of this system is strictly prohibited. Unauthorized users are subject to disciplinary proceedings and/or criminal and civil penalties under state, federal, or other applicable domestic and foreign laws. Individuals accessing LHI’s systems are subject to having all of their activities monitored and recorded by system personnel. Use of LHI systems evidences an express consent to such monitoring and agreement that if such monitoring reveals evidence of possible abuse or criminal activity, LHI’s systems personnel may provide the results of such monitoring to appropriate officials.
